Every person who comes to our practice has different needs, goals and perspectives unique to them. Before ever completing a comprehensive exam, our goal is find out more about you and your goals. We have discovered that most people want 3 things:

  1. They want to be HEALTHY (look pretty, chew well, have no pain)
  2. They want as little dentistry as possible over their lifetime
  3. They want to spend as little money as possible over their lifetime

Our exam process is specifically designed to help ensure that this happen. We call it your Personal Risk Assessment. Rather than simply looking for problems in your mouth and identifying “what’s wrong”; we look for good things to build on and, most importantly, identify risk factors which predict future disease.


Our comprehensive exam process takes you on a tour of your own mouth. It is different than most exams you have experienced in the past because we want you to be involved. After all, you are the one that will be caring for your mouth 363 days of the year, so you should be the expert. The guided tour is divided into four parts:

Within each category we will evaluate the present stage of health, risk factors that may cause future problems, and a thorough assessment of any current problems to identify the underlying cause. For many people, their entire life experience in the world of dentistry has been about mechanical solutions that did not change the underlying causes of disease. For example, they had a filling or a crown placed on a tooth but the reason for the cavity or the broken tooth was not addressed and eliminated. Because of this, they found that every few years, they were having to redo dental work with additional more expensive work. It is a dangerous downhill slide and can feel impossible to escape!


Once you see your current state of health and understand your risks, it is completely your choice how far and how healthy you want to become. For some, the end goal is for you to get as healthy as possible as quickly as possible and this is a stepping stone to a better life. For others, now may not be the right time but it will give you a future vision and path to pursue when you are ready.

At the end of our initial time together, you will leave with a Master Plan. This plan will focus on your choice of:

  • Pathwork dentistry – focusing only on the most urgent issues
  • Phased long term health – stabilizing disease, protecting teeth and gums from current risks, and preventing future disease
  • Rejuvenation – after disease is properly controlled, rebuilding the teeth to ideal form, function, and esthetics

When people say there is an app for everything, they weren’t kidding. Thanks to the KOIS CENTER, you can now learn how healthy your mouth is from the comfort of your own home. The Healthy Mouth App is a self-assessment app which can tell you what kind of condition your oral health is in by answering a series of “yes” or “no” questions. The questions are conveniently paired with pictures so you can identify which answer is most appropriate for you. The Healthy Mouth App conveniently breaks down your risk into four categories (Smile Characteristics, Gum Health, Bite and Jaw Health, and Tooth Health) very similar to the way we do.


You can expect the process to be quick, only about 5-10 minutes in fact. Here are the basic steps:

  1. Download the “Healthy Mouth App” from the IOS or Android store.
  2. Create and Register your account within the app.
  3. Answer the questions regarding your mouth.
  4. The apps algorithms will analyze your responses and provide you with a risk score.
  5. You will be directed to local offices (including ours) which can help you interpret your results.


Dr. Kolkman was awarded his Fellowship in the AGD by amassing over 1,000 hours of continuing education in the areas of cosmetics, sedation, implants, and biological dentistry. Additionally, our office is one of only two in NE Indiana certified by the IAOMT in safe mercury amalgam removal.
